Go to the Consent Document Templates screen as mentioned in the Pre-requisites section.
Click the Add Consent Document Template button as highlighted below.
Medic will display the below modal.
Enter the fields and upload the file of consent document.
Click the Save button as highlighted below.
Medic will save the consent document template.
Whenever you want to give any consent form to a patient just go to EMR > Patients, Open the patient’s profile there will be “Consents” from there you can upload to the patient profile.
