Go to the Consent Reports page as described in the pre-requisites.
Click the Sign button in front of the consent form, as highlighted in the below image.
The consent document will be opened, as shown below.
The patient should carefully read the consent document/form, and click the “Sign Document” button, as highlighted in the below image.
A box will open, as shown below.
Patients will sign in the designated area using a device (smartphone, tablet, etc.). They can also add other details, such as the date, by drawing them within the box along with the signature.
After signing, click the “Capture Signature” button, as highlighted in the below image.
A placeholder containing the signature will appear, as shown in the image below.
Adjust the placeholder to the required position for the signature, as seen below.
Click the “Save” button.
A dialog box will appear as shown below.
Click the “Submit” button.
A pop-up will display at the bottom-right for successful consent document submission, as shown below.
The signed consent document will then become saved in Care as well as Medic.
In care, the signed consent document can be viewed by clicking the “View” button from the Consent Reports page, as highlighted below.
The signed consent document will appear as seen below.
It can be viewed from medic as described in How to view consents, and will appear as shown below.
